These type vent covers are very common. This type is an inexpensive plastic cover and over the years will become very brittle. These are easily damaged with a slight hailstorm or fallen debris.

This innocent looking tub fixture had caused damage to the adjacent interior wall. The repairs to the wall had been completed and the tub area re-caulked, however the main cause is that the spout is not long enough and causes excessive splash back to the tub enclosure.
